Waiver

I, __________________________, acknowledge that I have voluntarily applied to participate in a trip operated by Katan Adventures. I understand that traveling with Katan Adventures may involve risks above and beyond those encountered on a more conventional trip, and that I am undertaking an adventure trip with inherent dangers. I am voluntarily participating on the trip with knowledge that travel to foreign countries and/or remote areas involves numerous risks and dangers including, but not limited to: the forces of nature; civil unrest; terrorism; roads; trails; hotels; aircraft, vehicles, boats or other means of transportation which may not be operated or maintained at standards common in the United States; accident or illness without access to means of rapid evacuation or availability of medical supplies; the adequacy of medical attention once provided; physical exertion for which I am not prepared; consumption of alcoholic beverages; or negligence on the part of Katan Adventures, or others. I have provided details of any pre-existing medical conditions to Katan Adventures representatives. I HEREBY AGREE TO BE RESPONSIBLE FOR MY OWN WELFARE, AND ACCEPT ANY AND ALL RISKS OF DELAY, UNANTICIPATED EVENTS, ILLNESS, INJURY, EMOTIONAL TRAUMA, OR DEATH AND VERIFY THIS STATEMENT BY PLACING MY INITIALS

I acknowledge that the cost of Katan Adventures trips is based upon trip participants executing this Release of Liability and Assumption of All Risks. Therefore, as lawful consideration for being permitted to participate on such trip(s), I HEREBY, FULLY AND FOREVER, RELEASE AND DISCHARGE KATAN ADVENTURES LLC AND ALL OTHER RELATED, AFFILIATED, ASSOCIATED OR OTHERWISE CONNECTED CORPORATIONS, PARTNERSHIPS, INDIVIDUALS AND OTHER ENTITIES AND ALL OF THEIR RESPECTIVE OFFICERS, DIRECTORS, EMPLOYEES, CONTRACTORS, AGENTS, SUCCESSORS AND ASSIGNS (COLLECTIVELY, THE “RELEASEES”) FROM ANY AND ALL COSTS (INCLUDING, WITHOUT LIMITATION, LEGAL COSTS), CLAIMS, DEMANDS, ACTIONS, CAUSES OF ACTION AND LIABILITIES WHATSOEVER FOR ANY AND ALL LOSSES, DAMAGES, DEATH OR INJURIES TO PERSONS OR LOSS OF PROPERTY, WHICH MAY BE SUSTAINED BY ME IN CONNECTION WITH OR IN ANY WAY RELATING TO THIS TRIP, DUE TO ANY CAUSE WHATSOEVER INCLUDING, WITHOUT LIMITATION, THE RECKLESS OR NEGLIGENT ACTIONS OR OMISSIONS OF THE RELEASEES OR ANY OTHER PERSON OR PERSONS .

I have carefully read and fully understand the contents and legal ramifications of this agreement as well as the conditions as stated under the heading Terms and Conditions, especially noting those regarding cancellation and refund policies, limitation of liability, and responsibility born by trip participants. I understand that this is a legally binding and enforceable contract and sign it of my own free will. I agree that if any portion of this agreement is found to be void or unenforceable, the remaining portions shall remain in full force and effect.
